What You will do:
Perform Retrospective/Prospective Coding: You will meticulously review patient medical records, including physician documentation, to accurately assign ICD-10-CM codes that map to appropriate HCC risk factors, ensuring documentation fully supports all coded conditions.
Uphold Documentation Integrity: You will actively communicate with providers and clinical staff through queries to clarify documentation, ensuring that the diagnosis coding is specific, complete, and meets the "MEAT" criteria for risk adjustment.
/>Ensure Compliance and Quality: You will maintain a consistently high coding accuracy rate and adhere strictly to all official coding guidelines, regulatory requirements, and client-specific standards.
Drive Audit Readiness: You will prepare and organize documentation and coding records to ensure the organization is continually ready for internal audits and external payer audits, such as the Risk Adjustment Data Validation program.
Stay Expert: You will actively track continuing education requirements to maintain professional coding credentials and stay current on annual coding changes and regulatory updates impacting risk adjustment models.
