Who you are:
To accurately assign ICD-10-CM codes that correspond to HCC categories, examine and evaluate medical data (physician notes, lab results, and diagnostic reports). Make that every coded condition is being actively managed by confirming that the doctor's paperwork satisfies the "MEAT" criteria to support the diagnosis. To keep each patient's Risk Adjustment Factor (RAF) score accurate, make sure all chronic conditions are recorded every year. To enhance the quality of medical records, find documentation gaps (such as missing diagnoses or undefined codes) and offer providers comments or questions.
To guarantee adherence to CMS regulations and avoid RADV audit failures, maintain a high degree of coding correctness (usually 95% or above).
