Your Duties:
Risk Adjustment Coding: To assign precise ICD-10-CM codes that correspond to HCC categories, examine and evaluate medical data (physician notes, lab results, and diagnostic reports).
paperwork Auditing: Make that all coded conditions are being actively handled by confirming that the doctor's paperwork satisfies the "MEAT" requirements to support the diagnosis.
RAF Score Management: To keep each patient's Risk Adjustment Factor (RAF) score accurate, make sure all chronic conditions are recorded each year.
CDI Collaboration: To improve the quality of medical records, find
documentation gaps (such as missing diagnoses or undefined codes) and ask providers questions or provide feedback.
Compliance & correctness: To guarantee adherence to CMS regulations and avoid RADV audit failures, maintain a high degree of coding correctness (usually 95% or greater).
